Meals That Meet Every Student’s NeedsEvery student deserves access to safe, nutritious meals that support their health and learning. OUHSD provides meal accommodations for students with documented medical needs, in accordance with federal and state guidelines.
 
 

How to Request a Special Meal

Step 1
Download and complete the form below:

Step 2
Have the form filled out by a California state-licensed healthcare provider

Step 3
Submit the completed form to the OUHSD Nutrition Services Department Attn: Nanci Miramontes, Food Service Director at nmiramontes@ouhsd.net

Step 4
Allow time for review and implementation; Nutrition staff will contact you if clarification is needed

Important Notes
  • Meal changes will only be made when the form is complete and signed by an approved healthcare provider

  • Accommodations apply only to students with an official diagnosis impacting dietary needs

  • Forms should be updated if a student’s condition changes
